Rosacea is often confused with acne because both cause red papules. However, the mechanism is completely different — and this changes the answer for LED therapy.
Why Rosacea Is Different from Acne
With acne, the leading causes are clogged pores and the bacterium C. acnes. With rosacea, the leading causes are vascular reactivity, impaired skin barrier, and neurogenic inflammation. There is no bacterium to be destroyed by blue light.

Heat — The Main Problem
Heat is one of the most well-documented triggers for rosacea — hot drinks, sauna, warm showers. If an LED device heats the skin, it can cause redness regardless of the wavelength.
This is why some people with rosacea report worsening after LED — often the problem is with heat dissipation, not the light itself.

Data for LED in rosacea is limited and weaker than that for acne or anti-aging. The overall picture:
